A Scrunty Piercing is an extremely rare piercing (few women even are anatomically able to get it) which passes horizontally through the urethra, sort of like a male frenum piercing would if it was pierced deep enough to be transurethral (frenums normally are not). This piercing will carry with it the same risks as a Princess Albertina does.
