Nailing is a form of CBT play involving hammering nails through the genitals.

It is not uncommon to use nails, even heavy nails in heavier CBT play. Nails through the scrotum produce surprisingly little long term damage, but nails through the testicles themselves or through the shaft of the penis are risky.
